Medical Evaluation - DMV has a five-page medical form that is designed to be filled out by your doctor. Most of the requested information consists of short responses with a few places for more detailed comments. Of particular importance are the questions:
Would the side effects from medication interfere with the safe operation of a motor vehicle?
In your opinion, does your patient's medical condition affect safe driving?
Have you advised against driving?
The medical evaluation is typically required for any medical condition, physical or mental. You may provide actual medical records or a letter from your doctor instead of the DMV form, but more is needed than simply a statement that “Patient is OK to drive.”
NOTE: Refusal to provide medical information may result in suspension (Vehicle Code §13801).
