When you receive a notice of re-examination or suspension
- If the suspension is immediate, request the first available
date for a hearing
- You may usually make the request by phone (Sacramento
(916) 227-2970).
- Note the date, time and the name of the clerk who took
your request.
- Confirm the phone request in writing by certified mail
including the date, time and clerk's name. For Sacramento area, mail to:

- Fax requests are not recommended.
- If there is no suspension pending or in effect, a written
request for your most convenient date is sufficient.
